Name Tag Order Form

We encourage everyone to get to know their fellow CLC members and visitors, and our name tags help us do just that! Name tags can be ordered from the website or at the Welcome Desk in the Crossroads. The cost is $10.00. If you want a new name tag and can't afford one, Pastor Ben will pay for it.

Tags are available in pin, clip, or magnet style. Please specify when ordering.

Orders for name tags will be placed twice monthly. Please be patient as the company works to get them done. It may take a few weeks from the time of your order to when we have them available.

New batches of name tags arrive weekly. Check for yours at the Welcome Desk!

Payment

The fee for a name tag is $10.00 per name tag.

After filling out the information below, please call the church office to process payment, bring payment to the office, or pay at the Welcome Desk in the Crossroads.

Filling Out Information

Include your name as you wish it on the name tag - first (or nickname) and last.

The 3rd line can be your home state or a fun fact (up to 12 characters) - like Raider Fan, Golden Knights, John 3:16, Book Fan, Quilter.
