Name Tag Order Form
We'd like everyone to have and wear a name tag - it helps build the community of Community Lutheran Church.
STEP 1
Payment
The fee for a name tag is $10.00 per name tag.
After filling out the information below, please call the church office to process payment, or bring payment to the office.
STEP 2
Filling Out Information
Include your name as you wish it on the name tag - first (or nickname) and last.
The 3rd line can be your home state or a fun fact (up to 12 characters) - like Raider Fan, Golden Knights, John 3:16, Book Fan, Quilter.
